As you prepare to make the arrangements for your loved one -- we found it helpful to have a check list for the items that are important to bring to the first meeting.

• Pre-planned arrangements (if made)
• Social Security # (preferably the card if available)
• Military discharge papers (form DD214).  We can assist you in applying for honors and benefits
• Birth certificate if available.  If not we will need date and place of birth as well as parents names including mothers maiden (birth) name
• 3-5 current photos to assist us the preparation process and for use on our website.
• A full compliment of clothing and any accessories (jewelry, glasses, etc.).
• Cemetery information and copy of the deed if available
